Facebook Friend Request Accepted From Person I Never Friended

Just today I signed into Facebook to find a new notification informing me that an Arabic-named person had accepted my friend request:

The only problem is, I never sent a friend request to this person. I rarely send friend requests out at all, and only to people that I talk to in person. I checked my friends list to make sure it wasn't an error and sure enough, my new "friend" was in there.

So what happened? I am extremely diligent when it come to online activity. I use no Facebook games or apps, I have AdBlock Plus and NoScript, and I have never visited that profile or any like it. I can only assume it was either a glitch on Facebook's side or this person found an exploit in Facebook to add a friend without consent.
Yes, I unfriended them. I also changed my password just in case by some insane circumstance it got phished. No obscure events, messages, posts, or anything though so that was likely unnecessary.
